CYS Services Enrollment Forms

What enrollment forms and information are needed to enroll? 

• Below are the forms and necessary information to enroll children 6 weeks through fifth grade and youth in grades six through 12. 

• Select the links to view the correct forms.  Print and complete prior to visiting the CYS Central Enrollment and Registration office.

• Other forms may be required at the time or enrollment. 
  
  

• Forms and documents to register children 6 weeks through fifth grade:

Form AE 608 and must include two emergency contacts*

CYS Health Assesment

Health Screening Tool DA Form 7625 and include shot records*

Fee Application DA Form 7652 and include LES

Additional information required to enroll children 6 weeks through fifth grade: (*Please note children cannot be enrolled into CYS Services without these items complete).

1. Two emergency contacts with current phone numbers.*

Forms to register youth 6th grade through 12th grade:

Teen Registration and Sponsor Consent and must include two emergency contacts.*

 Health Screening Tool DA Form 7625 is ONLY required if the teen has ADHD, asthma, allergies, or other medical conditions.  Shot records are NOT required.
